2nd Cohort of Teletherapy Program by Hear Me

MEMORANDUM OF UNDERSTANDING (MOU) BETWEEN HEAR ME AND EARTISTIC SDN BHD

The purpose of this MOU is to clearly identify the roles and responsibilities of each party as they relate to the designing, implementing, and evaluating of a project. In particular, this MOU is intended to clarify expectations of partnerships between Hear Me and provider.

This is the second cohort of the project and six families will participate in this project.

Each of six children will get 2 one –hour speech therapy sessions a month for 6 month with Dr Basyariatul Fathi Othman. Each session will be delivered via online through Zoom or any other application deemed necessary with approval of Hear Me.

IV. RESPONSIBILITIES OF HEAR ME UNDER THIS MOU

Hear Me shall undertake the following activities:

• To recruit six families to join the project based on criteria agreed between the two parties
• To forward payment to the provider of RM 100/per session
• To prepare parent kit that include media release form and parents agreement

V. RESPONSIBILITIES OF EARTISTICS UNDER THIS MOU

Provider shall undertake the following activities: 

• Speech Therapist to provide therapies to clients on the schedules mutually agreed with Hear Me participants

• Provide parents with report after end of 6 month program

• Develop an evaluation method to assess program
• Provide Hear Me with program evaluation report
